The How-To Guide is for practitioners who have questions on how to do assessments. It has the New York State Community Health Assessment (CHA) Guidance Documents for local health departments who have to submit community health assessments, description of a 10-step assessment process, and useful "how-to" (do assessments) links.
New York State Community Health Assessment (CHA) Guidance Documents
- 2010-2013 Community Health Assessment (CHA): Includes the Introductory Letter, CHA Guidance and Format document, and CHA Checklist/Index. The MPHSP, including the CHA must be submitted together, on or before July 31, 2009.
- Guidance Archives: Has 2005-2010 CHA, 2002 CHA Update, 2000 CHA Update, 1999-2004 CHA Guidance, 1999 Municipal Public Health Services Plan (MPHSP).
