Change in Method of Submitting Annual Report
As of May 1, 2008, nursing homes and adult homes must submit their annual report electronically on the Nursing Home Surveillance and Reporting system on the New York State Health Department of Health's (NYSDOH) Health Provider Network (HPN). Paper copies by mail or fax will no longer be accepted. The report may be accessed on the HPN at: https://commerce.health.state.ny.us/doh3/applinks/nuhsur/mainMenu.doc.
NYSDOH continues to make changes to enable adult day health care programs to submit the annual report electronically via the HPN instead of by mail or fax. Systems are currently in place for some adult day health care programs to submit their report electronically via the HPN. Adult day health care programs with existing HPN accounts may file electronically on the HPN nursing home survey at: https://commerce.health.state.ny.us/doh3/applinks/nuhsur/mainMenu.doc.
Adult day health care programs without HPN access are requested to submit their 2007-08 report form (DOH-4193) by mail to the address listed on the form. This form is available at http://www.nyhealth.gov/forms/doh-4193.pdf.
